Trust Review Questions


Trust Review:
1) Are you one of the following: individual trustee, corporate trustee,or the beneficiary of a trust?
2) Are you inquiring about a: life insurance trust, charitable trust, or family trust?
3) We'll need to know certain information about the trust. Please make sure to have the following information available when you contact us.

If family trust:
1) Are you the only trustee (if trustee)?
2) How many beneficiaries are named?
3) What is the approximate value of the trust?
4) Are the assets being managed according to your interests (if beneficiary)?
5) What was the year the trust was established?
6) What is the reason for reviewing?

If charitable trust:
1) How many trustees are there?
2) What is the approximate value of the trust?
3) Are the assets being managed according to your interests (if beneficiary)?
4) What year was the trust established?
5) What is the name of the primary charity?
6) What is the reason for reviewing the trust?

If life insurance trust:
1) Are you the trustee?
2) What is the date of birth of the insured (month and year only)?
3) What is the face value of the life policy?
4) How are the premiums paid (annually, semi-annually, quarterly, monthly, don't pay any premiums)?
5) What is the premium amount (only if not "you don't pay any premiums")?
6) Is the policy suppose to be "paid-up" (only if "we don't pay any premiums")?
7) What is the type of policy (whole life, universal life, variable life, term, second-to-die whole life, second-to-die universal life, second-to-die variable life, second-to-die term)?
8) What is the approximate cash value (if not term or not second-to-die term)?
9) Was the policy originally scheduled to be "paid-up" (if not term or not second-to-die term or not we don't pay any more premiums)?
10) Have the dividends decreased (if whole life or second-to-die whole life)?
11) Has the interest rate decreased (if universal life, variable life, second-to-die universal life, or second-to-die variable life)?
12) What is the reason for reviewing life insurance?
